Circle of Care is the geriatric care management division of the home care network of North Shore-LIJ Health System. Our goal is to provide excellent care coordination to the disabled and seniors throughout Queens, Nassau and Suffolk. We provide free, comprehensive evaluations to determine presenting problems, then help the senior and their family formulate a plan to keep them safe and comfortable in their own home as long as possible. We have an array of in-house services to support clients as well as a huge network of community resources to accommodate all our clients' needs. |
